Yes, I have first read and complied with your requirements for placing orders. I have also read and agree to the "Limit of Liability & Copyright Agreement" terms. I understand that any photo that is placed in my order upside down will be scanned upside down and put on the CD/DVD upside down as well. PHOTO MEMORY BOOK - Yes, I would like to order a Photo Memory Book. The charge is $59.95 per 1,000 images. Pages are double sided and bound in a hard cover book with 25 images per page in either a 5"x7" or 8½"x11" size. Image rotation is complimentary with an order of the Photo Memory Book. Additional copies of the same book are $24.95 per 1,000 images. (Example 1,700 pictures = $119.90 for the first book, $49.95 for each additional duplicate book) Price only valid for online ordering. Not valid in-store pricing.

Return Delivery Method, MUST choose one:

Return Shipping Option 1 - I would like you to return my entire order via USPS Priority Service and charge my credit card accordingly for shipping and handling. (minimum $19.95 per 1000 based on size of order, weight and destination. Typically the least expensive way to ship back.)

Return Shipping Option 2 - Split return delivery. For the quickest return of my scanned images, please FedEx Standard Overnight just the scanned digital discs for $37.50 and return my photos separately via USPS Priority Mail service with an additional, complimentary duplicate disc. (Minimum USPS Priority mail S&H is $19.95 per up to each 1,000 photographs, plus the additional $37.50 FedEx charge so my order is expedited so I can quickly get my scanned images ).

If choosing FedEx return of just your digital disc jpeg files, you must also choose one of the following Signature Delivery Authorizations:
Deliver without signature      Direct recipient signature      Any adult signature

Return Shipping Option 3 - I would like you to return my entire order via FedEx Domestic, 2-day delivery (P.O. Boxes not accepted) and charge my credit card accordingly for shipping and handling.
